Nipple Reduction

Overview of the surgery:
Nipple Reduction is a procedure used to reduce nipple size in a relatively simple procedure. Nipples that droop down or project too far out will be shortened. This is done by removing the tip of the nipple and then closing the new tip with stitches. Sometimes the skin around the neck of the nipple is removed and the tip is stitched to the bottom of the nipple. If the nipple is too wide or thick, but not too long, a pie-shaped wedge will be removed from underneath the nipple. This allows the nipple to be “taken in” and the circumference reduced. Dissolving sutures are used.

Duration of the operation:
Often, this can be combined with liposuction, tummy tuck, nipple reduction and other facial surgeries. The surgery takes 2 to 3 hours under general anesthesia with hospital confinement.

Post operative care:
You may need to return in about a week to have the stitches taken out, but usually this is not necessary as dissolvable stitches are often used. You will be able to shower the following day. Most patients can return to work and resume their normal activities within a day.

Recovery period:
Recovery time will depend on the patient, but most patients can resume their normal activity within a day or two. Patients can shower the day after surgery. Dissolving sutures are used.

Possible risk and complications:
Risks are inherent to any surgical procedure. The most common risks are swelling, bruising, bleeding, infection, fluid, numbness, or a change in sensation to the nipple; however they are rare and almost always temporary.
The most common risks particular to this surgery are minimal pain and swelling. Your surgeon will administer painkillers and anti – inflammatory medication so that any soreness and swelling will usually disappear by the second or third day. This treatment does not normally affect the ability to breastfeed.

Anesthesia use:
Nipple reduction technique can be performed under local anesthetic and is normally very effective.

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis

Preparation before Surgery
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Blood and urine samples will be collected for routine preoperative laboratory tests.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
  • You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
  • Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.

Breast Lift Surgery

Overview of the surgery:
Breast Lift surgery also called Mastopexy, removes excess breast skin to raise, reshape and firm your breasts. The incision of breast lift is placed around the areola, vertically down from the areola to the breast crease and horizontally along the breast crease. The surgeon raises and reshapes the breasts. The excess skin will be cut and the nipple and areola are repositioned to a natural, more youthful height. Breast lift can be done in conjunction with breast implant to increase breast volume.

Duration of the operation:

The surgery takes about 2-3 hours to perform.

Post operative care:
The patient needs to avoid lifting over head for 3-4 weeks. The patient should not resume strenuous activities till about 1 month after the surgery. A brassiere can be normally worn, but do not use brassieres with metal support during the first month

Recovery period:
After breast lift surgery, patients may experience discomfort, swelling, and bruising, which typically lasts from two to three weeks. Pain medications may be prescribed during the first week of recovery. Patients are required to avoid sleeping on their stomachs for the first two weeks, and are advised to sponge bathe until the sutures are removed.
During breast lift recovery, patients are required to wear a surgical bra for the first few days following surgery. This bra is then replaced with a support bandage or sports bra which is to be worn at all times for several weeks. The sutures are removed from the breasts after a week or two. Patients are normally able to return to work and light activities after about a week. However, during the first week, it is important to avoid bending over or performing any strenuous activities that put unnecessary pressure on the chest. Such activities can slow healing and lead to excess swelling and bleeding. Underwire bras should not be worn for the first month after surgery and instead a supportive bandage or bra should be worn at all times. There tends to be some numbness in the nipples and skin after breast lift surgery, which usually disappears in six weeks or so. Patients should not try lifting anything over their head for three to four weeks following surgery, and should avoid lifting anything heavy for four to six weeks.

Possible risk and complications:
The possible main risks are swelling, bruising, bleeding, infection, scarring and numbness, or change in sensation. Following the surgeon’s advice will reduce risks and complications.

Anesthesia use:

Breast lifted performed under general anesthesia.

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis

Preparation before Surgery
Your surgeon will examine your breasts, taking measurements and perhaps photographs for your medical record. The size and shape of your breasts, the quality of your skin and the placement of the nipples and areolas will be carefully evaluated. Information about any medical conditions, drug allergies treatments you received, previous surgeries including breast biopsies and medications that you currently take. It is important for you to provide complete information.

Augmented Mammoplasty (Breast Augmentation)

Overview of the surgery:
Augmented Mammoplasty (Breast Augmentation) is an operation to increase the size of the breasts using implants. The implants have a silicone shell that can contain either sterile saline/water or silicone liquid/gel. Incisions are made in inconspicuous areas to minimize visible scarring. Incision options include: Inframammary incisionTransaxillary incisionPeriareolar incision. Incisions vary based on the type of implant, degree of enlargement desired, your particular anatomy, and patient-surgeon preference. After the incision is made, a breast implant is inserted into a pocket either: Under the pectoral muscle (a submuscular placement), or directly behind the breast tissue, over the pectoral muscle (a submammary placement). The method for inserting and positioning implants depends on the type of implant, degree of enlargement desired, your body type, and your surgeon’s recommendations. Incisions are closed with layered sutures in the breast tissue and with sutures, skin adhesive or surgical tape to close the skin. Over time the incision lines will fade.

Duration of the operation:
The surgery requires 2 hours to complete.

Post operative care:
  • Move slowly and get some rest. Avoid activities that tend to wear you out. Recline your had slightly elevated above lower body.
  • Early ambulation is required to speed up healing process and improve circulation.
  • Eat a light diet at the day of surgery. Begin with liquids and progress to soup initially then advancing your diet to regular foods.
  • You may take shower 24 hours after the surgery. Gently rinse the breast area with water and pat dry. When showering remove the gauze dressing but leave the steri strips in place.
  • Avoid strenuous activities that can cause pain and discomfort.
  •  The incision will be covered with steri-strips and a small gauze dressing. It is normal to feel tired and sore for the next 2-3 days. It is normal to feel that the breast implants are not part of your body.  It takes 2-3 weeks for your beast implants to soften and feel more natural.
  • Bra: You do not need to wear a bra or may wear a loose fitting comfortable bra if desired.  Do not wear a push up bra for 6weeks.
  • Wound Care: Remove the gauze dressing before showering at least 24 hours after surgery. Steri strips will remain for the next 6 weeks. Avoid direct sunlight to the incision for 1 year. Use a sunscreen with zinc oxide with SPF 20 or greater.
  • Medications: Take all medications as prescribed.  Follow all instructions which may accompany the medications such as: Taking with food, avoid alcohol and driving restrictions.
  • Call the surgeon immediately for: Sudden onset of pain, fever, nausea, and vomiting, bleeding, dizziness, shortness of breath, rash, rapid heart beat or rapid breathing.
Recovery period:
You may go back to work in 2-5 days depending on how you feel.

Possible risk and complications:
The risks of breast implants and complications after breast augmentation include: unfavorable scarring, bleeding (hematoma), infection, poor healing of incisions, changes in nipple or breast sensation, may be temporary or permanent, capsular contracture, which is the formation of firm scar tissue around the implant, implant leakage or rupture, wrinkling of the skin over the implant, anesthesia risks, fluid accumulation, blood clots, pain, deep vein thrombosis, cardiac and pulmonary complications, possibility of revisional surgery.

Total Facelift

Overview of the surgery:
Total Facelift also known as rhytidectomy, is a cosmetic procedure to reduce sagging skin and lines by removing excess skin and fat, and tightening muscles
The incision of full facelift will begin above the hairline at the temple area, brought in front of the ear and looping behind the ear lobe and along the lower scalp. In full face lift the neck and face will be tightened altogether. The exact placement of incisions is determined upon the actual examination of facial structure. The patient may opt to do chin liposuction to enhance the result.

Duration of the operation:
A full facelift should take anywhere from 3 – 7 hours, depending on the patients requirements

Post operative care:
After the surgery, a patient will have to use cold compress for 2 days and avoid lying with the cheeks in contact with bed or pillow for 1 month to prevent hematoma formation. Some numbness may persist for 3-6 months. The patient should avoid any strenuous activities for at least 4-6 weeks following surgery.  This will give the body ample time to adjust as it continues on the healing process.

Recovery period:
Facelift sutures are removed between 5-7 days after surgery, and at this point swelling and bruising are becoming minimal. Makeup can usually be applied, and patients will start to resume some of their activities.

Possible risk and complications:
Full facelift surgery is a very common procedure and complications are rare, but can happen. This includes prominent, red scars, bleeding under the skin, infection, hair loss surrounding the scars, nerve damage, numbness, asymmetrical features and blood clotting.

Anesthesia use:
You may opt for either local or general anesthesia

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis

Preparation before Surgery

Temporal Lift

Overview of the surgery:
A temporal lift is a cosmetic surgery procedure designed to address signs of aging around the eye. This procedure can be combined with a forehead lift, eyebrow lift, and other cosmetic procedures.
The principle is to make a short incision ( long like the last finger) in the hair above the ear, parallel to the hair line but behind the anterior hairline, and to detach (undermine) in depth the anterior portion of the forehead and temple skin, eyebrow area and malar cheekbone skin areas. Then this area will be pulled and deep stitches will be placed to reposition. The incision and the scar are therefore hidden in the hair and not visible.

Duration o the operations:
The procedure usually takes is about 1 hour depending upon the extent of surgery.

Post operative care:
Recovery time and after-effects are simple and short and of various duration depending on individuals. Usually a week off social life is enough. The small clamps staples in the hair-bearing scalp are removed on the 8th day. Swelling of the upper part of the face and eyelids due to postoperative edema and ecchymosed will decrease gradually over the first 10 days, and then the residual edema will disappear in the next few months. It is usual the first weeks to have a bulge in front of the hairline: this will disappear spontaneously. Loss of hair is a rare event. Sensitivity and subtle motility of the scalp around the incision will return within a few months (with a possible pruriginous stage).

Recovery period:
We recommend only light activity for the first week to ten days following surgery. Most patients are able return to work 7-10 days following surgery with makeup easily covering any residual bruising.

Possible risk and complications:
Complications are very the unusual evolutions that require further treatment. The possible complications of the temporal lift are rare: Hemorrhaging, Hematomas and infection which may require an additional procedure and a specific medical therapy ,skin necrosis, and pain are very rare events, poor scaring, adherences, fibrous scar, visible scar, transitory paresis, or a paralysis of the frontalis or supercilii muscles  Impaired mimic or asymmetrical mimic, asymmetric eyebrows
Local alopecia, loos of hair local anaesthesia of the scalp, persistent asymmetry, itchiness
Psychological disorders (depression, psychosis), cardiovascular, immunoallergic or respiratory complications of anesthesia.

Anesthesia use:
The procedure usually requires general anesthesia.

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis.

Neck Lift

Overview of the surgery:
Neck lift is a minor surgical procedure that improves the appearance of a patient’s neck by removing excess skin and tightening the muscles underneath.
The doctor then usually makes small incisions behind the ears and underneath the patient’s chin, through which most of the surgery will be done. Reshaping the neck is a complicated process, and the surgeon will likely use a variety of methods to achieve the best possible result. The complexity of neck lift surgery means that it should only be trusted to an experienced, knowledgeable doctor.

Duration of the operation:
The surgery takes 1-2 hour’s average.

Post operative care:
Typically, neck lift surgery involves rapid recovery, though patients are advised to stay in bed for a few days after the surgery and avoid excess movement. Some may experience minor bruising and swelling, but these should subside within a week and should not be severe. Most doctors will have patients wear a compression bandage for about a week after the procedure to promote healing, and patients are advised to monitor their condition carefully for any complications arising from the surgery. Wear button-down shirt, so you don’t have to pull a shirt over your head, which could disturb the bandages.

Recovery period:
Most people can return to work in 10 to14 days. If you participate in other sports or are physically active, you will have to wait at least three weeks.

Possible risk and complications:
There are risks with any kind of surgery, and cosmetic procedures are no exception. In choosing to undergo a neck lift, the patient risks infection and possible allergic reaction to the anesthesia; the patient may also experience excessive scarring from the neck lift and other such complications. The occurrence of these effects is very rare, however, and can be reduced further with the choice of an experienced doctor. The patient should be sure to tell his or her doctor about any existing medical condition or medication being taken.

Anesthesia use:
Neck lift will be done under General Anesthesia.

  • Most people are advised to eat healthily, because the body will need nutrients to heal.

Mid Facelift

Overview of the surgery:
Mid facelift is a plastic surgery procedure that restores a smooth, youthful look to the lower eyelids and cheeks. During a mid face lift, small, deep incisions are made in the muscle tissue of the face. If the surgeon so chooses, an endoscope (a small, camera-like device) is inserted through the incisions to view the procedure internally. Once the incisions are made, the doctor adjusts the fat and muscle tissue, pulling up the middle of the face to smooth and tighten the skin. Finally, the incisions are sutured.

Duration of the operation:
The midfacelift usually takes around 1 hour.

Post operative care:
Although discomfort should be minimal immediately following face lift surgery, patients will not be in a condition to drive home. Their head will be bandaged and they may still be experiencing the effects of anesthesia. They should therefore arrange for transportation to and from the treatment center with a friend or family member. During the week following face lift surgery, they should also plan on maintaining a relatively light schedule. Often, having someone to help with everyday activities such as cooking and cleaning can make the recovery period much easier.

Recovery period:
Most patients can return to work from a mid face lift in 10 days to two weeks. For the first few days or weeks, depending on your procedure, only walking and mild stretching are appropriate physical activities. Vigorous exercise, heavy housework, and sexual activity should be avoided for at least the initial two weeks after surgery. Alcohol, saunas, and steam rooms should be avoided for several months. During the face lift recovery period, extra rest is very important to promote proper healing.

Possible risks and complications:
Aside from the risks inherent in any surgical procedure, risks specific to face lift surgery include:
·        Injury to the nerves that control facial muscles - this is usually temporary.
·        Problems with healing of the skin - this is common for patients who smoke.
·        Scarring - there will be some scarring, although the surgeon will always try to hide the face lift scars.
·        Crust on the incisions - this will be temporary.
·        Numbness or tingling around the incision areas - this should be temporary.
·        Skin or hair loss - this is rare.
·        Skin discoloration - this is a rare condition that can continue for several months.
Some of the most common postoperative face lift complications are:
·        Reaction to the anesthesia – some face lift patients experience nausea and general sickness after receiving anesthesia; this is a short-lasting side effect and usually diminishes completely after a few hours.
·        Hematoma – if bleeding occurs under the skin, a hematoma can develop, leaving the area painfully swollen; an operation is required to stop the flow and remove the blood.
·        Bleeding – although some light, postoperative bleeding is normal, uncontrolled bleeding is dangerous and can lead to painful hematomas.
·        Infection – typically occurring in a specific area, symptoms of infection are swelling, sensitivity, redness, and localized heat. Unless it is severe, an infection can be relieved with antibiotics.

Anesthesia use:
Mid face lift surgery procedures can be performed using local anesthesia although general anesthesia is also an option. During more intensive procedures, general anesthesia may be required.

  • Hair should be washed thoroughly the day before face lift surgery to minimize the risk of infection. Additionally, alcohol should be avoided the night prior to the procedure.
  • Patients should also make sure to get a good night’s sleep, since it is important to be well rested during face lift surgery. Some cosmetic surgeons may give patients additional instructions.
